An individualized mRNA neoantigen therapy plus Keytruda cut the risk of recurrence or death versus Keytruda alone in 1,137 patients with completely resected stage IIB-IV melanoma, according to a prespecified interim analysis of the Phase III INTerpath-001 trial. Merck and Moderna also said the combo significantly improved recurrence-free survival and distant metastasis-free survival, setting up potential regulatory filings.

The result is one of the clearest late-stage validations yet for personalized mRNA cancer vaccines in the adjuvant setting.
INTerpath-001 tested intismeran autogene (V940/mRNA-4157), which is built from each patient's tumor mutational signature to generate a tailored antitumor immune response.
For clinicians, the readout suggests a possible new post-resection option for high-risk melanoma beyond PD-1 monotherapy.
Safety was reported as consistent with prior studies of the regimen.
